Why do I need a Thyroid test?

Your thyroid is more important than you think. The thyroid is a small, butterfly-shaped gland located at the base of the neck and is an integral part of your endocrine system. The endocrine system is made up of glands which regulate the function, growth and development of virtually every cell, tissue and organ in the body by secreting chemicals called hormones directly into the bloodstream. When endocrine disorders develop, too much or too little hormone is secreted by a specific gland because the gland stops functioning properly due to illness, surgical removal or natural causes.

Thyroid dysfunction occurs when the thyroid produces either too much thyroid hormone, which causes your body’s systems to speed up (hyperthyroidism), or too little hormone, which causes your body’s systems to slow down (hypothyroidism). Both negatively impact your overall health and well-being.

Untreated thyroid disease may lead to elevated cholesterol levels and subsequent heart disease, as well as infertility and osteoporosis. Research also indicates a strong genetic link between thyroid disease and other autoimmune disease, including certain types of diabetes, arthritis, and anemia.

Thyroid disorders affect an estimated 200 million people worldwide, and an estimated 27 million Americans. More than half remain undiagnosed. In fact, thyroid disease is more common than diabetes and heart disease.

What is included in the tests?

TSH (Thyroid Stimulating Hormone). A TSH test measures the amount of thyroid stimulating hormone in your blood. TSH is produced by the pituitary gland and tells the thyroid gland to make and release the hormones Thyroxine (T4) and Triiodothyronine (T3). Triiodothyronine (T3) and Thyroxine (T4) help control your body’s metabolism. Nearly every cell in your body depends on normal T4 and T3 levels to maintain its optimal function.

Free T4 (Thyroxine). T4 is the predominant thyroid hormone produced by the thyroid gland. Free T4 measures the T4 hormone that is not bound to protein; therefore it is ready for use by the body. Measuring free T4 rather than total T4 is a more accurate assessment of thyroid status.

Free T3 (Triiodothyronine). T3 is the most active thyroid hormone. Some T3 is produced by the thyroid, but most of the T3 comes from conversion of T4 to T3 which occurs in body but outside the thyroid. Free T3 measures the available (non-protein bound) T3 hormone and its measurement is very useful in assessing thyroid balance, particularly in hyperthyroidism.

Thyroid Antibodies. Thyroid Antibodies are tested for evidence of autoimmune thyroid diseases, such as Hashimoto’s or Graves’ disease. Thyroid antibodies may remain positive for years, and do not provide an indication of whether the person has normal or abnormal thyroid function. Furthermore, some patients with Hashimoto’s disease may have negative levels of circulating antibodies, and conversely, patients with positive levels of thyroid antibodies may never develop thyroid disease during their lifetime.

Do I need to fast before taking the test?

No. Fasting is not required before a thyroid function lab test.

Do I need approval from my insurance company before testing?

These services are not covered by insurance. Therefore, you do not need approval from your insurance company before having a thyroid test.

Do I need a referral from my health care provider in order to use ANY LAB TEST NOW®?

No, it is not necessary to have a referral to use ANY LAB TEST NOW®.

What types of payment do you accept?

We accept cash, and all credit cards like Visa, MasterCard and American Express.

How will I receive my test results?

Test results will be available within 72 hours.

How does thyroid hormone testing differ from a saliva test or a finger-stick blood test?

Saliva testing is an unproven technology and is not medically recognized by most accredited medical communities for thyroid assessment. Because the sensitivity of saliva testing hasn’t been clearly established, saliva testing has proven to be inconsistent and unreliable, not to mention more expensive. Finger prick spot blood tests are also unreliable and expensive and do not adhere to any medical standards for testing thyroid function.

Graves’ disease is the most common form of hyperthyroidism, occurring when your immune system mistakenly attacks your thyroid gland and causes it to overproduce the hormone thyroxine. This higher thyroxine level can greatly increase your body’s metabolic rate, which may affect you in numerous ways, from your moods to your physical appearance.

SYMPTOMS MAY INCLUDE:

Anxiety

Irritability

Difficulty sleeping

Fatigue

A rapid or irregular heartbeat

A fine tremor of your hands or fingers

An increase in perspiration

Sensitivity to heat

Weight loss, despite normal food intake

Brittle hair

Enlargement of your thyroid gland (goiter)

Change in menstrual cycles

Frequent bowel movements

It’s also fairly common for your eyes to exhibit mild signs of a condition known as Graves’ ophthalmopathy.

Graves’ ophthalmopathy may cause these mild signs and symptoms:

Excess tearing and sensation of grit or sand in either or both eyes

Reddened or inflamed eyes

Widening of the space between your eyelids

Swelling of the lids and tissues around the eyes

Light sensitivity

There’s no way to stop your immune system from attacking your thyroid gland, but treatments for Graves’ disease can ease symptoms and decrease the production of thyroxine.

In Hashimoto’s disease, also known as chronic lymphocytic thyroiditis, your immune system attacks your thyroid gland. The resulting inflammation often leads to an underactive thyroid gland (hypothyroidism). Hashimoto’s disease is the most common cause of hypothyroidism in the United States.

Hashimoto’s disease does not have unique signs and symptoms. The disease typically progresses slowly over a number of years and causes chronic thyroid damage, leading to a drop in thyroid hormone levels in your blood. The signs and symptoms are mainly those of an underactive thyroid gland (hypothyroidism).

Hyperthyroidism (overactive thyroid) is a condition in which your thyroid gland produces too much of the hormone thyroxine. Hyperthyroidism can significantly accelerate your body’s metabolism, causing sudden weight loss, a rapid or irregular heartbeat, sweating, and nervousness or irritability.

SYMPTOMS MAY INCLUDE:

Fatigue

Feel nervous, moody, weak, or tired.

Have hand tremors, or have a fast or irregular heartbeat, or have trouble breathing even when you are resting.

Sweat a lot, and have warm, red skin that may be itchy.

Have frequent and sometimes loose bowel movements.

Have fine, soft hair that is falling out.

Lose weight even though you are eating normally or more than usual.

In addition, some women have irregular menstrual cycles or stop having periods altogether, and some men may develop enlarged breasts.

The symptoms of hyperthyroidism are not the same for everyone. Your symptoms will depend on how much hormone your thyroid gland is making, how long you have had the condition, and your age. If you are older, it’s easy to mistakenly dismiss your symptoms as normal signs of aging.

If you do not treat your hyperthyroidism, you may:

Lose weight because your body’s metabolism is faster.

Lose weight because your body’s metabolism is faster.

Have heart problems such as rapid heart rate, atrial fibrillation, and heart failure.

In rare cases, hyperthyroidism can cause a life-threatening condition called thyroid storm, which develops when the thyroid gland releases large amounts of thyroid hormones in a short period of time. Thyroid storm usually happens after you have had a serious infection or you have had a lot of stress in your life.

Treatment:
The good news is that accurate thyroid function tests are available to diagnose hyperthyroidism and several treatment options are available if you have hyperthyroidism. Doctors use anti-thyroid medications and radioactive iodine to slow the production of thyroid hormones. Sometimes, treatment of hyperthyroidism involves surgery to remove part of your thyroid gland. Although hyperthyroidism can be serious if you ignore it, most people respond well once hyperthyroidism is diagnosed and treated.

Hypothyroidism (underactive thyroid) is a condition in which your thyroid gland does not produce enough of certain important hormones.

Symptom may include:

Fatigue

Sluggishness

Increased sensitivity to cold

Constipation

Pale, dry skin

Muscle weakness

Heavier than normal menstrual periods

Brittle fingernails and hair

Depression

When hypothyroidism isn’t treated, signs and symptoms can gradually become more severe. Constant stimulation of your thyroid to release more hormones may lead to an enlarged thyroid (goiter). In addition, you may become more forgetful, your thought processes may slow or you may feel depressed.

Treatment:
The good news is that accurate thyroid function tests are available to diagnose hypothyroidism, and treatment of hypothyroidism with synthetic thyroid hormone is usually simple, safe and effective once the proper dosage is established.

Thyroid function tests are a series of measurements of different aspects of thyroid hormone presence in the blood stream.

TSH (Thyroid Stimulating Hormone)
Because of the normal feed-back loop, TSH rises whenever the thyroid gland fails to produce sufficient thyroid hormone. For the same reason, whenever there is too much thyroid hormone present in the serum, the brain responds by lowering the TSH. It is elevated in hypothyroidism and depressed in hyperthyroidism.

T4 (Thyroxine)
This is a measure of all of the active form of thyroid hormone in the blood. 95% of the total is bound to plasma proteins, particularly thyroid binding globulin (TBG).

It is elevated in hyperthyroidism and depressed in hypothyroidism. It is usually slightly elevated during pregnancy because of increased levels of TBG.

T3 (Triiodothyronine, T3-RIA) by radioimmunoassay
T3 (Triiodothyronine, T3-RIA) by radioimmunoassay T3 is metabolically a more active form of thyroid hormone than T4, but its’ effects are of shorter duration. It is elevated in hyperthyroidism and depressed in hypothyroidism.

T3 Uptake
This is a measure of the unbound throxine binding globulins in the blood (unsaturated with thyroid hormone). The less thyroid hormone that is present, the more unsaturated TBG is present. This is an indirect measure of thyroid hormone levels. It has nothing to do with T3, despite the name.

Free T3 (FT3, Free Triiodothyronine T3)
Free T3 is one of the determinations used to measure thyroid function. T3 is mostly bound to plasma proteins, and the remainder (FT3) is unbound. It is elevated in hyperthyroidism and depressed in hypothyroidism.

Free T4 (FT4, Free Thyroxin)
Most thyroid hormone is bound to plasma proteins. Some (about 5%) is not bound (“free”) and available to serve a metabolic role. Free T4 is the most accurate assessment of thyroid hormone levels, because it avoids changes in plasma protein binding capabilities. It is elevated in hyperthyroidism and depressed in hypothyroidism.

FTI (Free Thyroxine Index, FTI, T7)
The FTI is obtained by multiplying the (Total T4) times (T3 Uptake) to obtain an index. The FTI is considered a more reliable indicator of thyroid status in the presence of abnormalities in plasma protein binding. It is elevated in hyperthyroidism and depressed in hypothyroidism.

Furthermore, some patients with Hashimoto’s disease may have negative levels of circulating antibodies, and conversely, patients with positive levels of thyroid antibodies may never develop thyroid disease during their lifetime
